Feline Calicivirus (FCV): Symptoms, Limping Syndrome, and the Rare Deadly Strain

Most cats that catch calicivirus go through a rough week of sneezing and mouth ulcers and bounce back fine. In genuinely rare outbreaks, the same virus mutates into something else entirely, a systemic disease with a mortality rate that rivals some of the most feared illnesses in veterinary medicine. The gap between those two outcomes is worth understanding.
What ordinary calicivirus actually looks like
Feline calicivirus (FCV) is, alongside feline herpesvirus, one of the two viruses behind most feline upper respiratory infections. International Cat Care describes the typical case as sneezing, nasal discharge, and conjunctivitis, usually accompanied by something herpesvirus less commonly causes: ulcers on the tongue, gums, or hard palate that can make eating visibly painful.
The limp that isn't a leg injury
One of calicivirus's stranger presentations is what's informally called "limping syndrome." Certain strains trigger joint inflammation and fever, and an affected cat, often a kitten shortly after a vaccine or natural infection, can suddenly limp or seem reluctant to walk with no history of trauma at all. It typically resolves on its own within a few days as the immune response settles, but it's a good example of why an unexplained limp in a young cat is worth a vet visit rather than an assumption of an injury that never happened.
Why the vaccine doesn't fully prevent infection
Calicivirus is genuinely tricky from an immunology standpoint. A 2025 review in the journal Animals describes FCV as existing as a "quasispecies," meaning new genetic variants are constantly emerging, and that antigenic variability among strains can reduce how well any single vaccine cross-protects against every version circulating. Vaccination still matters a great deal: it reduces viral load and shedding duration and generally makes illness milder, it just isn't a guarantee against ever catching some strain of the virus.
The rare, genuinely dangerous form
Very occasionally, calicivirus mutates into virulent systemic feline calicivirus (VS-FCV), which behaves nothing like the ordinary version. The same 2025 review reports mortality rates for VS-FCV outbreaks ranging from roughly 22% up to 86% depending on the specific outbreak, with affected cats developing severe systemic inflammation, clotting abnormalities, and multi-organ failure rather than a cold. These outbreaks tend to cluster in veterinary hospitals and shelters, where close contact between cats allows a highly contagious mutated strain to spread rapidly before it's identified. It's rare, but it's the reason a cat with an unusually severe or fast-progressing "cold," especially with swelling of the face, paws, or fever well beyond a typical case, needs urgent veterinary evaluation rather than a wait-and-see approach.
Reducing the risk in multi-cat settings
Because FCV can survive on surfaces for up to 10 days, far longer than herpesvirus, thorough cleaning with a disinfectant proven effective against it (not all "pet-safe" cleaners are) matters more for calicivirus than for most other feline viruses. Isolating new or sick cats before introducing them to a household or shelter population, and not sharing bowls or litter boxes between cats during an active outbreak, meaningfully reduces spread.
